You've assumed the respondent knows what character design is. Better get everyone on the same page with maybe an introductory sentence or two detailing the sorts of things generally included with character design. It only takes a small number of confused people to skew a survey's results.

Granted, you do put some details within the survey questions here and there, but by then it's too little, too late.
